General Requirements

• ☐  LTA-approved supervisor name, NRIC and registration number
• ☐  Public liability insurance certificate (covering public road works)

Plans & Drawings

• ☐  Site location plan (key plan on street map)
• ☐  Detailed site plan showing work area and road reserve boundary
• ☐  Existing road, footway and drainage shown on plan
• ☐  Road diversion plan or TMP (where applicable)
• ☐  Road signage layout plan with sign types
• ☐  Pedestrian detour route shown
• ☐  Work zone layout with safety barrier positions

Method Statement

• ☐  Work method and sequence
• ☐  Working hours (day / night / 24-hour)
• ☐  Traffic impact during works
• ☐  Traffic control method (flagmen, lights, signs)
• ☐  Pedestrian and cyclist safety provisions
• ☐  Emergency vehicle access maintained

PE / Authority Requirements

• ☐  PE endorsement of TMP (if required by LTA for scope)
• ☐  PE letter or certification for significant road diversions
• ☐  Coordination with utility owners if applicable
