  • Project title and exact site address
  • Client, contractor, QP, PE / Professional Engineer and consultant contact details
  • Proposed work dates, working hours and duration
  • Existing authority references, if any
  • Urgency and target submission date

Drawings and site records

  • Location plan and site plan
  • Existing road, footpath and frontage layout
  • Proposed works drawings
  • Photos of affected road / footpath / access
  • Survey or as-built drawings, where available

Traffic and construction information

  • Method statement and construction sequence
  • Traffic Control Plan / road diversion sketch
  • Road signage, temporary signage, barricade and lighting proposal
  • Material delivery and vehicle access plan
  • Pedestrian, cyclist and emergency access consideration

Authority and stakeholder records

  • LTA comments or previous submissions
  • Permit / NOA reference number, if any
  • Other agency comments involving PUB, NParks, SLA, URA, HDB or JTC; LTA approved contractor / LTA approved supervisor records where applicable
  • MCST / owner / managing agent requirements
  • Known site constraints and nearby sensitive users
